Quick answer

  • Sukuna’s true form is his original Heian-era body: four arms, four eyes, a face mouth and a second mouth on his stomach. He takes it back during the Shinjuku Showdown, transforming mid-fight against Kashimo (Ch. 237) after absorbing his own mummified corpse (Ch. 222).
  • The manga explains what the extra parts are for: two hands can hold a hand sign while the other two fight, and the stomach mouth chants incantations nonstop without straining his heart or lungs (Ch. 238).
  • Chapter 257 reveals that Sukuna ate his twin in the womb. Many readers link this to his extra arms and eyes, but the manga never states that link directly.
  • The look comes from the Ryomen Sukuna of the Nihon Shoki (720 AD): a man from Hida with two faces and four arms who used two bows at once.

Sukuna’s true form is the body he had in the Heian era: four arms, four eyes and two mouths, one on his face and one across his stomach. He only gets it back late in the story, during the Shinjuku Showdown, when he transforms in the middle of his fight with Hajime Kashimo (Ch. 237).

Why four arms? The manga gives a practical answer: two hands can keep a hand sign up while the other two fight. It also reveals that Sukuna ate his twin in the womb, which many readers see as the source of his extra limbs. The design itself comes from a real legend in the Nihon Shoki.

This article contains spoilers up to the end of the manga (Chapter 271). Anime episode numbers are overall numbers (S1 = 1–24, S2 = 25–47, S3 = 48–59). The true form has not been animated yet; it appears well after where Season 3 Part 1 ended.

What does Sukuna’s true form look like?

Sukuna’s original body is that of a tall, muscular man with:

Feature Detail
Arms Four, two on each side
Eyes Four. The two right eyes sit on a bony growth on the right side of his face (Vol. 29)
Mouths Two: the normal one and a large mouth across his stomach
Markings The black tattoo lines that appear on Yuji and Megumi when Sukuna takes over are his real markings
Clothing Usually a black haori over his shoulders, presumably because that torso is hard to dress

The tattoos are a useful clue. When Sukuna controls a two-armed vessel, the lines around the upper arms and wrists appear doubled. That is the four-armed body showing through.

Is the four-armed form his “Heian form”?

Yes. English-speaking fans use true form, Heian form and four-arm form for the same body. It is the form Sukuna had over 1,000 years ago, when he was a human sorcerer, not a cursed spirit.

The manga says that in this form, armed with the cursed tools Kamutoke and Hiten, Sukuna wiped out the Fujiwara clan’s elite forces, including the Sun, Moon and Stars Squad and the Five Empty Generals (Ch. 238).

One point that confuses people: the version of Sukuna in Yuji’s body is not a weaker “form” of the same body. It is Yuji’s body with Sukuna in control. The same goes for the Megumi version. Only the four-armed body is Sukuna’s own.

Sukuna spends most of the series in other people’s bodies. The table shows each stage.

Stage Body When
Heian era His own four-armed body Over 1,000 years ago
After death Split into 20 fingers (cursed objects); his corpse kept mummified by Tengen beneath a mountain in Hida Ch. 220
2018 Yuji Itadori’s body, after Yuji eats a finger Ch. 1 (Episode 1)
Shibuya Incident Still Yuji’s body, after he is force-fed many more fingers Ch. 112–119 (Episodes 39–41)
Culling Game Moves into Megumi Fushiguro’s body Ch. 212
Before Gojo fight Eats the fingers Uraume collected; uses his mummy in place of the one finger still missing Ch. 222
Vs. Kashimo Resumes incarnation and turns Megumi’s body into his true form Ch. 237

In Chapter 222, Uraume brings three more fingers and apologizes for not finding the last one. Sukuna says it doesn’t matter: his mummified Heian body can make up for one finger’s share of his power.

He holds the transformation back through the whole Gojo fight. Only when Kashimo lands a lightning strike he can’t avoid does Sukuna use it, both to heal and to fight at full strength (Ch. 237). Kashimo looks at the new body with his X-ray vision and calls it perfect (Ch. 238).

Why does Sukuna have four arms?

There are two answers: what the arms do, and where they came from.

What the extra arms and mouth are for

Chapter 238 explains this directly:

  • Two extra hands can form hand signs while the other two keep fighting. Hand signs normally tie up a sorcerer’s hands.
  • The stomach mouth can chant incantations continuously without putting strain on his heart or lungs.
  • No drawback. Despite the strange anatomy, his organs and body work normally.

The manga calls this the biggest advantage a jujutsu sorcerer could have. You can see it in action right away: Sukuna holds Kamutoke in his mouth, fights Kashimo with all four arms, and uses the stomach mouth to chant for the world-cutting Dismantle (Ch. 238). Kashimo dies in that chapter.

The twin he ate in the womb

In Chapter 257, a flashback set before the Gojo fight shows Sukuna explaining Yuji’s origins to Uraume. Sukuna reveals that he ate his twin in their mother’s womb. His mother was starving, and he did not want to starve.

That chapter has Sukuna say the twin was reborn as Jin Itadori, Yuji’s father. Volume 29 then corrected this: the twin was reborn as Wasuke Itadori, Yuji’s grandfather (Vol. 29). So Sukuna is, in a strange sense, Yuji’s great-uncle.

Does eating the twin explain the four arms? The manga never says so directly. It is the most common fan reading, since the extra arms, eyes and mouth look like a second body merged into his. But treat it as a theory, not canon.

What the manga does confirm is that his body made him an outcast. Sukuna tells Kashimo he was shunned early in life (Ch. 237). In the final chapter, Mahito suggests he lived for revenge on the people who scorned him for being abnormal, and Sukuna doesn’t deny it (Ch. 271).

The real legend: Ryomen Sukuna in the Nihon Shoki

Sukuna is named after a real figure from Japanese legend. The Nihon Shoki (Chronicles of Japan, completed in 720) has a short entry for the 65th year of Emperor Nintoku’s reign. It describes a man named Sukuna in Hida Province (today’s northern Gifu Prefecture):

  • One body with two faces, facing opposite ways
  • The heads joined at the crown, with no nape
  • Arms and legs on each side: four arms and four legs
  • Strong and fast, with a sword on each hip
  • He used two bows and arrows at once with his four hands
  • He ignored the emperor’s orders and raided the people, so the court sent a commander, Takefurukuma, to kill him
Nihon Shoki Sukuna JJK Sukuna
Faces Two, back to back One face, but four eyes
Arms Four Four
Legs Four Two
Weapons Swords and two bows Cursed tools Kamutoke and Hiten; a flame arrow (Divine Flame)
Link to Hida From Hida Province His mummy was kept in Hida (Ch. 220)
Fate Killed by the imperial court Defeated by Yuji and the sorcerers (Ch. 268)

The Official Fanbook says the JJK character got the name because his look and strength resemble the legend. He is not meant to be the mythical Sukuna himself.

A villain in the chronicle, a hero in Hida

The legend has a second side. Along the old road through Hida and Mino, local temple traditions describe Ryomen Sukuna very differently:

  • Senkō-ji (Takayama): its founding record calls him an incarnation of the Kannon bodhisattva who founded the temple.
  • Zenkyū-ji (Takayama): also says it was founded by Ryomen Sukuna and keeps a statue of him.
  • Hiryūhō-ji (Seki): says a two-faced, four-armed man subdued a poisonous dragon on the mountain.

Most of these records were written in the Edo period or later. Historians read the Nihon Shoki entry as a hostile description of a local Hida leader who resisted the Yamato court, but the details remain uncertain.

Sukuna’s technique: Shrine and the flame arrow

The true form changes how Sukuna fights, but not his technique. His innate technique is Shrine (御廚子, Mizushi).

Move What it does Source
Dismantle (解, Kai) Invisible ranged slash; works on curses, people and objects Ch. 119 (Episode 41)
Cleave (捌, Hachi) A slash that adjusts to the target’s toughness and cursed energy; outside his domain it needs touch Ch. 119; Ch. 246
World-cutting slash Dismantle aimed at space itself, modeled on Mahoraga’s adaptation; killed Gojo Ch. 236
Divine Flame (竈, Kamino) A flame arrow, activated by saying “Open” (開, Fūga); usable only after Dismantle and Cleave Ch. 116; Ch. 259

Older guides often say the fire might be a second technique. Chapter 259 settles it: Divine Flame can only be used after Dismantle and Cleave, which ties it to Shrine.

Malevolent Shrine: why the domain is so big

Sukuna’s domain expansion, Malevolent Shrine, doesn’t close its barrier. It builds the domain in open space instead (Ch. 119, Episode 41).

  • Leaving an escape route acts as a binding vow.
  • In return, the sure-hit area grows to a maximum radius of nearly 200 meters.
  • Inside it, everything is cut nonstop with Dismantle or Cleave, depending on its cursed energy.
  • The manga compares building a domain without a barrier to painting on air without a canvas.

This is what turned a large part of Shibuya to rubble against Mahoraga (Ch. 119, Episode 41).

How Sukuna’s story ends

The true form doesn’t save him. After Gojo, Kashimo and many others, Yuji’s repeated Black Flashes and Yuji’s own domain expansion wear Sukuna down. Nobara’s Resonance on a remaining finger pins him in place (Ch. 267).

In Chapter 268, Yuji forces Sukuna out of Megumi’s body. Megumi survives. Yuji offers to let Sukuna live inside him again. Sukuna refuses and tells him not to underestimate him, because he is a curse. Then he crumbles away.

In the final chapter, Sukuna meets Mahito in the passage where souls circulate. He admits he had other choices in life and says he would choose a different path next time. He walks on with Uraume (Ch. 271). In the real world, his last finger sits unsealed in a thermometer box. Yuji says it is now harmless, more of a charm against curses (Ch. 271).

FAQ

Not yet. Season 3 Part 1 ended on March 26, 2026, well before the Shinjuku Showdown chapters where the true form appears (Chapter 237 onward).

Is Ryomen Sukuna a real legend?

Yes. The Nihon Shoki (720) describes a man called Sukuna in Hida with two faces and four arms who used two bows at once and was killed by the imperial court. Temples in Gifu Prefecture remember him as a hero instead.
